Fill out all the requested information below and then click on the Submit button.
After submitting the form, you will see a message on your screen that says, "Thank you. Your information has been submitted." If you don't see the Thank You message, scroll up to the top of the page to look for the message.
If you are still concerned that your form did not go through, you can fill it out and submit it a 2nd time - duplicate entries will be deleted.

- If you need to make a change to your choices, just submit a 2nd form. Only the most recent form for each person will be retained.
- Please, do NOT call or email to ask if we received your form or to request a manual change to your entry - we do not have the bandwidth to either search for your form or to respond to those messages.
- Submit the raffle form ONLY if your intention is to actually take a class in the Spring Session. If you just want to be notified about future classes, join our mailing list instead by clicking here.
